McLean Home Care and Hospice Earns CHAP Accreditation Again

Patricia Adams, McLean Administrator for Home Care & Hospice, and Sara Szafranski, Director of Home Care, share the news of the CHAP accreditation. The Community Health Accreditation Program, Inc. (CHAP) announced this week that McLean Home Care & Hospice has been awarded CHAP accreditation for Home Health and Hospice Standards of Excellence. McLean has earned this designation consecutively since 2006. CHAP accreditation demonstrates that McLean Home Care & Hospice meets the industry’s highest nationally recognized standards. Rigorous evaluation by CHAP focuses on structure and function, quality of service, human and financial resources, and long term viability.

Only 5,000 agencies of the more than 16,000 in the US go through the extensive accreditation process. Of the fourteen VNAs and Hospice programs serving the Farmington Valley, McLean is the only agency that is CHAP accredited. Visit www.CHAPinc.org for an agency locator.

Adams noted, “McLean goes above and beyond to assure that we follow best practices in our professional every day. Earning the CHAP accreditation again demonstrates that the team has achieved our goal. I am extremely proud of them as the process is rigorous. The visiting team from CHAP that spent a week with us was very complimentary. They went on home care and Hospice visits, audited many records, and interviewed staff and patients. We incorporate their recommendations into our on-going quality improvement process.”

What does this mean to a family who needs to choose a home care agency to provide skilled care to their loved one?  Adams explains “It is one more assurance that you have chosen the best. I have no doubt that the quality that helps us earn the CHAP accreditation directly relates to the fact that 91% if our Medicare patients would recommend us to family and friends, the highest rating of any visiting nursing agency in the 14 towns we serve. Now we need to help families exercise their right to choose. Often when someone is leaving the hospital, they agree to any VNA not understanding there are many VNAs and Hospice programs in the area and the range and quality of services varies widely.”

McLean Home Care & Hospice serves people in their homes, in assisted living facilities and nursing home in the Farmington Valley, through Windsor, Bloomfield West Hartford and Greater Bristol areas.
